Bus/Passenger Van Request Form

  1. Buses and Passenger Vans are assigned based on a first-come, first-served basis; therefore proper travel planning is recommended.
  2. Department Head Approval is required when utilizing departmental fund/budget codes for travel purposes. Please obtain approval before requesting a bus or van for all travel needs.
  3. Only University Bus Drivers are allowed to drive the buses and passenger vans.
  4. Departments are responsible for the University Bus Driver's overnight lodging and other costs associated with their travel (ex: toll and bridge fees, parking costs, etc.)
